Librarians and Root Cause Analysis: what can we do (beyond lit provision) and what would be learn?

Nice discussion on Wachter's World on the value of root cause analysis (RCA) and meeting weekly to discuss a reported incident in depth. Despite the lack of evidence that a UCSF librarian takes part in the process (too bad!) I would like to hear how our group here might not only contribute to the process, but learn from it as well!
