I responded to a listserv post recently on visuals for a patient safety display oriented to clinicans. I know there are others out there, but wanted to share my "top of mind list" should anyone here be putting together a display on safety for patient safety awareness week (March 7-13).

Nice discussion on Wachter's World on the value of root cause analysis (RCA) and meeting weekly to discuss a reported incident in depth. Despite the lack of evidence that a UCSF librarian takes part in the process (too bad!) I would like to hear how our group here might not only contribute to the process, but learn from it as well!

Speech writing is in lots of ways similar writing a paper, apart from that there is no penalty for spelling and punctuation mistakes. You should not try to write words in your speech that you are not relaxed pronouncing or don't know the meaning of because it can lead to a less fluently delivered speech.
